The Ryanair Window: Why the NTSB Is Relitigating a 2018 Southwest Tragedy

A Boeing 737NG operated by Ryanair subsidiary Malta Air suffered an uncontained engine failure shortly after takeoff from Thessaloniki, Greece, on July 10. Debris punctured the fuselage and shattered a cabin window partially ejecting a 61-year-old Serbian passenger who was saved only by his wife and fellow travellers holding him inside. The aircraft returned safely to Thessaloniki.

The incident has prompted the US National Transportation Safety Board to assume a lead investigative role and forced the FAA into a public reevaluation of its response to the near-identical fatal Southwest Airlines Flight 1380 event in 2018. For Asian operators still flying large numbers of 737NGs the story raises uncomfortable questions about the pace of safety upgrades on one of the world’s most common narrowbody workhorses.

Echoes of Southwest 1380 parallels are unmistakable. In April 2018, a Southwest 737-700 experienced a left-engine fan blade failure. Debris damaged the engine cowling and inlet which then struck the fuselage and blew out a window. A passenger was partially ejected and died the first fatal accident on a US passenger airline in nearly a decade. The NTSB blamed a fatigue crack in the fan blade and criticised the design of the fan cowl which failed to contain secondary damage.

Following that accident, Boeing developed modifications to strengthen the inlet cowl, fan cowl and exhaust nozzle. The FAA eventually mandated changes, but the compliance deadline for operators is July 31, 2028, a full decade after the Southwest tragedy. Thousands of 737NG aircraft worldwide, including many in Asian fleets SpiceJet, legacy Malaysia Airlines aircraft and others are still operating with the pre-modification configuration.

The current incident and regulatory response in regard to the Ryanair Malta Air event involved the right No. 2 CFM56-7B engine. Initial reports and imagery indicate fan blade damage and cowling penetration with debris striking the fuselage. The NTSB is now deeply involved at Greece’s request and FAA Administrator Bryan Bedford has publicly stated the incident is triggering a 'full reevaluation' of the agency’s post-2018 actions.

The 737NG remains in widespread service globally even as the 737 MAX dominates new deliveries. Any systemic weakness in engine containment or cowl design exposed in 2018 and apparently recurring in 2026 demands urgent attention.

Asia still operates hundreds of 737NGs. While many carriers have modernised, others continue using the type on high-frequency regional routes where reliability and rapid turnaround are critical. A slower-than-expected rollout of the FAA-mandated nacelle modifications means a non-trivial portion of the global 737NG fleet may still be flying with the original cowl design when the 2028 deadline arrives.

Regulators and operators in India, Southeast Asia and beyond will be watching the NTSB/FAA findings closely. The incident underscores a broader truth in aviation safety. A legacy fleets require continuous scrutiny. Fan blade-out events are statistically rare but when they occur secondary damage to the airframe can turn a contained engine failure into a cabin decompression emergency.
